Individuals new to our Center are asked to submit completed forms prior to scheduling an appointment. You may access all our necessary forms via the forms link or we will mail them to you. Return the completed forms by fax to (336) 334-4475 or mail them to:

Insurance:
The Center is not a participating provider with private insurance companies therefore, we are considered “out-of-network”. If asked, we will provide you with a walk-out statement detailing your visit.

Reduced fees for some services are available to those who qualify.  This does not include equipment.

The Center is a Medicare Part B provider for select hearing services; speech-language services are not currently covered at this Center by Medicare.

Payment methods include cash, personal checks, Credit Cards: MasterCard and Visa.mc_vs_accpt_h_023_gif

Adverse Weather and Emergency Closings Policy: If the university is closed due to adverse weather or emergency evacuation the Center will not open for services. If there is a delay in opening the university the Center will follow the same schedule of opening. If you have questions about whether or not your appointment stands please call the office at (336) 334-5939.

Parking and Directions to the Center:

The Speech and Hearing Center is located on the campus of UNCG.  The Center has nine parking spaces for clinic parking only that are conveniently located adjacent to the Ferguson Building.  These spaces are identified by signs labeled “Reserved Parking Clinic” (see map).  You must have a valid parking pass to park in these clinic parking spaces.  Please request a pass in advance of your appointment or arrive early so that one may be issued to you.  The circle drive is for drop offs only.  Street parking along Highland Avenue may be available, but is limited to two hours and is not guaranteed.
